S/C Belt Problems...any help

Well, guys I have gone through 3 new belts in the last 3 weeks. I'm not really sure whats going on. The belt walked off the backside of the supercharger pulley, so i took out the tensioner and spacered it out a little bit. Now it's coming off of the front of the pulley. But it's actually staying completely on the crank pulley. It's just too weird...any ideas...Here are some pics

Something has been installed offset. You can see by the fraying around the edges of the belt, all you are doing is shifting the weight from the front to the rear.

Either the drive and supercharger arent spaced correctly so they line up (talking spaced from the motor so the belt runs straight), or something is twisted. Have you got a method of adjusting the position of the supercharger? the bracket hasnt bent or twisted has it? or even come loose for that matter?

Was it a DIY install or did you pay someone to do it?

It looks like the belt is crooked/shifted compared with where it should be on the tensioner. As said before, check the brackets and make sure nothing is twisted. All the pullys and what not are tight and straight too, right?
